Participation Agreement Including Assumption of Risk, Waiver of Liability, Indemnification, Media Consent & Training Policies

Please read this Agreement carefully before submitting. Consent to Participate I certify that I am eighteen (18) years of age or older and voluntarily consent to my own participation in basketball training, skills sessions, and related physical activities provided by Sage Hoops.

I certify that I am the parent or legal guardian of the Minor named below, a minor under the age of eighteen (18), and consent to the Minors participation in basketball training, skills sessions, and related physical activities provided by Sage Hoops. Assumption of Risk I understand that participation in physical and athletic activities involves inherent risks, including but not limited to bodily injury, illness, permanent disability, paralysis, property damage, or death, and I knowingly and voluntarily assume all such risks on behalf of myself and/or the Minor, whether arising from participation, instruction, facilities, equipment, or activities associated with Sage Hoops. Release and Waiver I hereby release, waive, discharge, and hold harmless Sage Hoops, its owners, coaches, employees, agents, contractors, volunteers, and representatives (collectively, the Releasees) from any and all claims, demands, actions, or causes of action arising out of or related to any injury, loss, or damage sustained by me and/or the Minor as a result of participation in the activities, including those caused by the negligence of the Releasees, to the fullest extent permitted by law. Indemnification I agree to indemnify and hold harmless Sage Hoops from any claims, actions, damages, liabilities, costs, or expenses, including legal fees, arising from or in connection with my participation or the Minors participation in the physical recreational activities provided by Sage Hoops. Medical Treatment In the event of any injury or illness during participation in these activities, I authorize Sage Hoops and its staff to administer or seek medical treatment for myself and/or the Minor, including but not limited to arranging for emergency medical services, transportation, or hospitalization, if deemed necessary, and I understand that I am solely responsible for any medical expenses incurred as a result of such treatment. Severability I understand that if any provision of this waiver of liability is found to be unenforceable or invalid, the remaining provisions shall remain in full force and effect. Safety and Equipment I understand and agree that parents should ensure that their child comes prepared for each session with appropriate sports attire and equipment, such as proper footwear, and that they should emphasize the importance of following safety guidelines and instructions provided to minimize the risk of injuries. Media Consent I grant Sage Hoops permission to photograph, record, or capture my likeness and/or the Minors likeness during training sessions or events for promotional, educational, or marketing purposes, including use on websites and social media platforms, and I understand that no compensation will be provided for such use and waive any right to inspect or approve the final media content.

Scheduling Policy I understand that Sage Hoops operates on a scheduled appointment basis and that reserved session times are held specifically for each athlete.

Cancellation Policy I agree that cancellations or rescheduling requests must be made at least twenty-four (24) hours prior to the scheduled session time.

Late Cancellation and No-Show Policy I understand and agree that if a session is canceled with less than twenty-four (24) hours notice, or if I fail to attend a scheduled session without prior notice, the session will be forfeited and no credit, refund, or rescheduling option will be provided.

Trainer Cancellation Policy I understand that Sage Hoops reserves the right to cancel or reschedule sessions due to facility conflicts, emergencies, illness, or circumstances beyond its control, and that in such cases a credit or rescheduled session will be provided. Governing Law I understand that this Agreement shall be governed by and interpreted in accordance with the laws of the State of Idaho.
